BEHAVIORAL CONTRACT EMPOWERING POTENTIAL HOUSING - 3-DAY NOTICE TO PERFORM COVENANT OR QUIT 

I, Client first name  Client last name, am a resident in Empowering Potential Recovery Home.  This is a sober living residence that provides an authentic peer to peer-based recovery support system.  While I reside here, I am encouraged to actively participate in my recovery and am expected to adhere to the posted rules/guidelines implemented by management.

I am being placed on this Behavioral Contract/given this 3-day notice to perform covenant or quit for the following reason(s):

(i.e. use mind-altering substances while living in this sober home, failing to perform my chores, not following other rule (curfew, guests policy, etc.).  Please be specific as to what rules where broken!

I will set these immediate goals to help me change my action/behavior:

You have 3 business days from the date of this notice to cure this covenant or quit (move out) - you may be given grace to continue staying at Empowering Potential Housing if you immediately make changes as spelled out above.  

If I am unable to meet my goals, I will be asked to leave Empowering Potential Housing, (without a refund of any monies paid if applicable).
