Living Amends Sober Living Scholarship Contract

Living Amends is a 501 (c) 3, a non-profit dedicated to supporting people who are working to achieve long term sobriety. We provide financial scholarships to those who are experiencing financial hardships during the first few months of sobriety. We select applicants who have made the initial financial and emotional investment into their sobriety through completing 30, 60, or 90-days in a residential substance abuse treatment facility. Living Amends provides financial support during the aftercare phase of recovery, otherwise known as extended care or sober living.

Living Amends mission is to assist individuals by offsetting living costs so they may continue to keep their recovery on the forefront and work through the 12 steps

This agreement is between Client first nameClient last name (Recipient) and Living Amends and stands as a written committment to ones own personal recovery and the understanding that we must give it away to keep it.

All parties agree to the terms of this agreement as-is.  This agreement is a living document and may be modified by Living Amends with input and feedback from recipient, the board and his/her sober living house leadership.

Recipient may submit an application for up to 3 months. A new application must be submitted monthly. The amount allotted will be as follows:

75% applied to first months rent, 25% to be paid back within 12 months

50% applied to second months rent, 25% to be paid back within 12 months

25% applied to third months rent, 25% to be paid back within 12 months

By signing below, recipient understands and agrees to the following conditions:

• Recipient agrees to obtaining a sponsor and working all 12 steps.
• Recipient agrees to all rules and regulations in the sober living.
• Recipient agrees to attend 3-5 meetings per week.
• Recipient agrees to pay back Living Amends 25% of awarded scholarship monies within one year to help the next person in need.
• All checks will be mailed directly to address of sober living owner's address of choice
