- I understand that if I exceed 24 hours with no contact with any house member/s that a missing person report may be filed with the local police and that if I am gone from the house over 72 hours that my things may be packed up and I lose my house status/spot/room/bed.

-  I agree to attend a minimum of 3 recovery activities weekly (this is in addition to any aftercare recommendations and/or government mandates such as but not limited to IOP, PO visits, & drug court sessions)

- I understand that my attendance at a weekly house meeting with the house members is mandatory to go over house events and provide information on my own recovery progress.

- I understand that my $300 move in fee in non-refundable.

- I understand that I will contribute my share of living expenses of $200 weekly is paid in advance at the beginning of each week and if I incur a balance equal to or greater than $400, I will be expelled from the house.  The House may vote to extend a member's stay, however under no circumstances will the member balance increase.  A minimum of weekly living expenses must be paid within 2 days from the weekly incurred day of each week for extension by house vote to remain valid.  Weekly living expenses include utilities, internet & cleaning supplies. It does not include food or personal hygiene items.

- I understand that if I am expelled from the house, move out on my own cognizance and/or exceed 24 hours with no contact with the house that my next of kin/emergency contact will be notified.

- All house members must provide a UA upon request without exception.

By continuing, you agree that your electronic signature is the legally binding equivalent to your handwritten signature. Whenever you execute an electronic signature, it has the same validity and meaning as your handwritten signature. You will not, at any time in the future, repudiate the meaning of your electronic signature or claim that your electronic signature is not legally binding.
