Recuperative Care / Short-Term Post Clients
Program Expectations & Safety
Empowering Potential Housing maintains an abstinence-supportive culture and believes that refraining from mind-altering substances can support healing of physical, emotional, and mental conditions.
Participation in Recuperative Care is not conditioned on sobriety. Clients who prefer a recuperative care setting with a different clinical or operational approach may request assistance with referral to an alternative recuperative care provider by contacting Rachel at 619-500-3987.
Program Purpose
-The primary goal of Recuperative Care is medical stabilization and recovery following illness, injury, or hospitalization.
This program does not provide substance use treatment.
Clients are expected to comply with basic house safety rules designed to protect the health and safety of all residents.
Substance Use & Safety Response
If substance use occurs during a Recuperative Care stay, staff will assess for immediate medical or safety concerns and may support access to:
-Hospital care
-Detoxification services
-Other appropriate medical settings
When necessary to maintain health, safety, or program operations:
-Temporary off-site placement may occur
-Temporary off-site placement is not considered a discharge
-Bed holds may be offered at the program’s discretion based on clinical and operational factors
Services Provided During Recuperative Care
Clients may receive support including:
-A safe place to rest and recover
-Care coordination with hospitals and healthcare providers
-Medication support (reminders and refill assistance; no dispensing)
-Follow-up appointment coordination and discharge planning
-Daily wellness and recovery check-ins
-Connection to primary care, behavioral health, housing, and benefits
Grievance Rights
Clients have the right to file grievances or appeals with their health plan at any time.
Empowering Potential Housing also encourages clients to raise concerns directly with staff so issues may be addressed promptly. Clients may contact the business line at 619-500-3987 to speak with James or Rachel. This is voluntary and not required prior to contacting a health plan.
Services are delivered consistent with *CalAIM Community Supports guidelines and Medi-Cal member rights.
